Notice: Effective on your September 2003 invoice (August 2003 usage), a $0.99 monthly surcharge will be assessed per account per month in order to recover costs the Company incurs with regard to TeleRelay service, National Number Portability and Federal Regulatory fees. This surcharge will appear as a separate line item on your invoice.
